  • Live Answering Service is offered ONLY to individuals, business entities and service providers that are 100% medical.
  • Plans Offered:

  • Acceptance is subject to approval.

TERM:

  • NO LONG TERM CONTRACT
  • Minimum Term is one (1) Billing Cycle.
  • One (1) Billing Cycle is four (4) weeks or twenty-eight (28) days.
  • The Term selected guarantees that rate for that term period.
  • Term choices are:
    1 = 4 weeks
    3 = 12 weeks
    6 = 24 weeks
    13 = 52 weeks
    26 = 104 weeks
    39 = 156 weeks
  • Save more by signing up for longer term.
  • Posted rates are the current rates.

NOTE:

  • All services below require manual activation and provisioning.
  • Your order date will be different from the Activation Date.
  • The Date Range of the Term for Service(s) appearing on this Order Form when the order is placed will be adjusted to your actual Activation Date.
  • Your Activation Date will then be the start of your actual Term's Date Range.
  • Billing Period starts on your Activation Date.
  • You will be notified when service is activated.

  • Your Recurring Invoice is due upon receipt.
  • Late Fee of 2% assessed on any outstanding balance left after the end of that Billing Cycle.
  • Preferred method of payment is Autopay.

- Recurring Base Charges are due before start of the next term.
- More savings on Base Charges for longer Terms selected.
- Usage Charges on activity.
- This recurring invoice covers only the Base Charges.
- A separate invoice will be sent every 4 weeks (28 days) after the end of each Activity Period for all Usage Charges incurred during that period.

    - One (1) Primary Subscriber.
    - Calls answered 24/7.

- Usage Charges on activity are per occurrence/time.
- Option to include additional Subscribers.

- BASE and FIXED Plan: Calls are NOT answered Monday to Friday during the day for eight (8) hours on pre-arranged designated times.
- For calls that needed to be answered outside of on any of those pre-arranged designated hours, the rate charged for each day or portion of that day answered will be at 1/25th of your recurring Base Charges.
- You will receive a separate invoice.
